Chronic illness is by definition, long lasting and at times severe. Chronic illnesses are large contributors to mortality, morbidity, disability and increased hospital admissions. They also make up a significant proportion of individuals using primary health care services. In Australia, almost half of all deaths are caused by the 12 leading chronic illnesses which include a range of physical and mental illnesses. These include coronary heart disease, stroke, lung cancer, colorectal cancer, depression, diabetes, asthma, chronic obstructive pulmonary disease, chronic kidney disease, oral diseases, arthritis and osteoporosis.
