This unit applies theory and skills of disease state management into the management of common chronic medical conditions: asthma, chronic obstructive pulmonary disease, hypertension, hyperlipidaemia, heart failure, blood clotting disorders, diabetes and obesity. Medication management refers to recognised guidelines and patient management principles refer to contemporary Australian research. The unit culminates in an assignment requiring students to extrapolate their skills and knowledge to design a service for patients with another chronic medical condition. Practical and future application of these services is discussed. -- Course Website
